As healthcare delivery evolves from an acute care setting to non-acute care to H@H, the pharmacist’s role also needs to remain at the forefront of direct patient care. IDNs need to integrate pharmacy leadership and workforce into planning, implementation, and patient care management for hospital-at-home services. CMS’s Acute Hospital Care at Home program allows eligible patients to be treated for acute illnesses in their homes, fostering a migration of clinical services outside of the walls of the hospital. More than 60 acute conditions such as heart failure, asthma, pneumonia, and chronic obstructive pulmonary disease (COPD) can be safely managed from a patient’s home with proper monitoring and treatment protocols. During this session, we will discuss eligibility requirements, patient monitoring, clinician support, and other aspects to implement a successful acute hospital care at home program.
