Achieving clinical integration involves the facilitation and coordination of care across a variety of settings and situations. The goal is to achieve care that is safe, timely, efficient, patient-focused, and affordable. This track will provide insight into improving clinical integration strategies and aligning incentives with behaviors for success. Discussions will cover a variety of timely topics, including implementing increased data standardization practices, leveraging clinical learnings from the COVID-19 pandemic, and realigning your organization based on changing patient expectations.

The COVID-19 pandemic deeply impacted how and where you provide patient care. Driven by patient expectations of omnichannel care and access as well as expanded reimbursement guidelines, care episodes are increasingly occurring outside the hospital.
