Description

This education session aims to equip clinical pharmacy professionals with an understanding of the various population health factors and social determinants that impact healthcare outcomes in different populations. Additionally, the session will cover topics such as health disparities, health literacy, care access, testing, diagnostics, and vaccines - providing attendees with tools to better serve diverse patient populations. This knowledge will enable clinical pharmacy professionals to better understand targeted interventions to improve health outcomes and promote health equity.

Learning objectives:

1. Develop an understanding of the social determinants of health and how they impact healthcare outcomes across a variety of populations, including vulnerable and marginalized populations.

2. Describe the impact of vaccines and diagnostics and the critical importance of community buy-in.

3. Explain the impact of the RSV virus and how vaccines contribute to population health and avoidable admissions.

4. Examine the importance of point-of-care testing/rapid diagnostics
